Applying for a New Passport
The new passport process is for anyone who has either: are applying for a first-time US passport, are looking to renew a 5 year US passport, or are looking to renew a 10 year US passport that has been expired for 5 years or longer.
To obtain a new passport book or card, you will need:
- A completed application form DS-11. Form can be found here.
- Proof of citizenship. Visit the Department of State to view a list of relevant documents.
- A passport photo. (Vernon Township is able to take photos of applicants age 5 and older for a small fee.)
Renewing Your Passport
The renewal passport process is ONLY for those who are renewing a 10 year passport that either has not yet expired or expired less than 5 years ago.
To obtain a renewal, you will need:
- A completed application form DS-82. Form can be found here.
- Most recent passport.
- Passport photo. (Please note that Vernon Township does not have the ability to provide digital photographs, only printed.)
Passports for Children Under 16
Applicant must be present. Applications for children 4 and under must provide their own photos. Parents apply on behalf of the child; both parents must sign application in the presence of an agent or they must present a notarized Statement of Consent form or a notarized statement of why the second parent could not attend.
Statement of Consent Form should include the child's full name, birth date, and name of consenting parent and a photocopy of the front and back of the non-appearing parents identification. Learn more from the Department of State about obtaining and renewing passports for children under 16.
Applicants Age 16 to 17
Applicants age 16 to 17 go through a different process from children 16 and under. Visit the Department of State for more information about applying for a passport for this age group.
